The applicant (“You”) understand and acknowledge the following statements and terms of the Canadian Academy of Medical Acupuncture INC. (the “Academy”) membership: 1) The Academy is a voluntary acupuncture organization. It is not an acupuncture regulatory body. 2) Membership is optional. That is, You do not have to join. 3) There are some insurance companies that accept the “Academy” membership as a requirement for payment to acupuncture providers currently; namely, WSIB Ontario, Great West, Claim Secure, Empire Life, Chamber of Commerce, Johnson Group, Sun Life, Standard Life 4) Greenshield currently only accept chiropractors, but may give individual consideration to professionals who have 200 hours of acupuncture training. 5) This list of insurance companies may change from time to time or may no longer be valid once the College of Traditional Chinese Medicine Practitioners and Acupuncturists of Ontario is established. 6) There are continuing education requirements needed for Academy membership renewal. 7) You may not use the title “acupuncturist” in Ontario or advertise yourself as an acupuncturist as this title reserved for practitioners who are members of the College of Traditional Chinese Medicine Practitioners and Acupuncturists of Ontario . 8) You will only use the acupuncture practitioner title permitted by your College. 9) The use of the title “Certificant of the Canadian Academy of Medical Acupuncture” or “CCAMA” is permitted by the Academy when your membership is active and all annual dues are paid. 10) The fees, terms and conditions for joining or renewing the membership with the Academy may change from time to time without prior notice to You. 11) You have read, understood and accepted the terms and conditions on our web sites www.acupunctureacademy.ca and www.eacupuncture.ca. 12) Refund: For those members enrolled in a multi-year discounted membership, the membership fee for the first year and for each year thereafter is calculated at the regular membership rate for refund purposes. For refund requests made within the first membership year, the amount to be refunded is the difference between the membership fee paid and the membership fee for the year or years elapsed at the regular membership rate. For example, if You paid $250 for a 5 year membership and request a withdrawal or cancellation of your membership the next day, You will be refunded $55. If You request a refund of your membership fee after the first year, You will have a refund balance of $0. 13) When the Academy is no longer relevant or active, we will refund you the difference based on the policy from the preceding paragraph (12). 14) You should get advice from your legal counsel if You do not understand any of these terms and conditions.
